Personal Supports
Personal Supports are person-centered services delivered in the individual’s preferred manner with a focus on gaining, maintaining, or improving their independence in a home or community setting. Services provided include supervision, personal care assistance as needed, and opportunities for activities. Supports are individually designed to respond to the needs and desires and to reflect an appreciation of the individual’s unique personality. Services may include but are not limited to In-home skills, Community Integration, and Navigation as well as Personal Care.
- In-home Skills emphasizes improvement in task completion at home. It may include budgeting and money management, assistance in completing homework, maintaining their bedroom or apartment/home, cooking, personal care assistance, laundry, household chores, etc.
- Community Integration advances skills in grocery shopping and planning, obtaining haircuts or styling, public transportation, and safety training.
- Personal Care will aid individuals to learn how to complete activities of daily living. This may include bathing, dressing, hygiene techniques, and instrumental daily techniques. When an individual cannot prepare meals independently, it may also include hot meal preparation and training.
The objectives of the program are designed to assist in the development of skills, which will enable the individuals served to live in the least restrictive environment with the proper supports that meet their needs. Personal Support Services areas include, but are not limited to:

- Transportation/Mobility Training
- Behavior Support
- Case Management
