Destiny’s Group Home, Inc. will create a family consultation plan, which will be based on family-oriented goals to benefit the participant.

Family Consultation Plan Includes:

A participant’s family shall be trained by a qualified licensed or certified professional to provide intensive one-on-one interventions with the participant in the following areas:

• Daily Living Skills
• Behavior Shaping and Management
• Self-Direction
• Socialization
• Mobility
• Money Management
• Habilitation (e.g., communication, sensory integration)

Families shall receive in-person, individualized consultation services in the participant’s home, DGH’s main office, or in a community setting.

Families will be instructed in treatment regimens, behavior interventions and modeling, skills training, and in the use of instructional materials.

Parents will be provided with training updates.

DGH, Inc. will return calls/emails within 24 hours.

DGH, Inc. will address any parents’ concerns.
