Day Habilitation
The objectives of the program are designed to assist in the development of skills, which will over time enable the individuals served to acquire supported or volunteer work opportunities according to their individual goals and preferences. Skill development areas include, but are not limited to:

Individual preferences/desires are the first consideration in program placement. This program emphasizes the essential aspects of person-centered planning and self-determination through promoting communication and choice making while exploring interests. The program values providing a variety of opportunities to meet and address the individual needs, preferences, and interest areas. Supervision is provided based upon the individual’s needs as outlined in the Person-Centered Plan. On-site support from staff will be provided to DDA individuals in areas of life skills management, community integration, transportation-mobility services, task analysis-training, positive behavior supports, health-fitness activities/training, case management coordination, and social interactive training.
Setting:
A licensed site is designated for Day Habilitation Services.
